Not another digital copy of a paper form.

Fertilité already had Spanish and English consent documents. What it needed was a way for patients to complete them online, without printing, scanning, or updating the same authorization at every visit.

The focus was communication consent: patients needed to understand the clinic's virtual assistant, choose which channels were acceptable, and sign once. Staff needed a record they could actually find and download.

Clarity for the patient, visibility for the clinic. The interface treats each permission as an explicit choice, not a blanket checkbox.

Spanish and English views, independent yes/no choices for messaging, AI-assisted calls, and email, optional marketing consent, and a touch-friendly signature pad.

02 / STAFF

Find the record when it matters.

The password-protected dashboard lets staff search submissions, review an individual record, and download its signed PDF. Notification support is configured for new submissions.

The visible form is only one layer. A useful consent workflow also needs validation, durable storage, and a staff retrieval path.

Separate consent by channel, independent marketing opt-in, and a guardian option keep decisions legible.

Each submission stores a signature and signing time; staff can generate a signed PDF record on demand.

Deployable infrastructure

The app runs as a separate Docker project on the clinic's existing VPS, with persistent SQLite storage and an automated image build.

The delivered application connects a patient-facing consent experience to a staff-accessible record. It was intentionally built as a one-time authorization, not a per-visit re-signing system.
